The Third Street Family Health Services Administrative Offices serve as the organization's primary business office and official mailing address. This location houses our executive leadership and administrative departments that support the delivery of care across our health centers throughout north central Ohio.

The Administrative Offices are the appropriate location for:

• Medical records requests
• Legal notices and service of process
• Billing and insurance correspondence
• Human Resources and employment-related inquiries
• Vendor and business correspondence
• General administrative communications
