We look forward to servicing you at our Main Third Street Family Health Services Campus. Our headquarters is just off of Park Ave West on the hill where Western Ave and West 3rd Street meet. The main entrance to our parking lot is off West 3rd Street. Patient entrances can be found on each side of the building including accessible ramps and handicap parking near the entrances.
